LETTERS REQUESTED FOR PURPOSES OTHER THAN REFERRAL TO A CONSULTANT/HOSPITAL.
Please fill in the form on the right if you are looking for a letter from your G.P. for ANYTHING other than referral to a Consultant or for a hospital service. Letters cannot be requested over the phone. You MUST use this form. The doctors will not take calls about letters or discuss them in person during consultations.
You must put as much information as possible into each section as if you do not include information you want in your letter here it will not be put in the letter. The doctors will not discuss letters over the phone so it is in your own interest to make sure you have provided all the information requested.
There is a fee of €20.00 payable in advance for all letters. You can drop cash to the surgery in an envelope with your name, address and phone number on it our you can ring in and pay with a card over the phone. Your letter will not be started until payment has been received.
Letters for non-medical reasons, e.g. housing, sun-beds, travel will take at least 2 weeks to be completed.
